One-on-One Survivor & Caregiver Connection Program

Sometimes, all it takes is one conversation with someone who “gets it” to change everything. The Peer Support Network at A Stroke of Grace pairs stroke survivors and caregivers with trained peer supporters who have walked a similar path. These personal, one-on-one connections offer emotional support, encouragement, and real-life guidance from someone who’s been where you are.

How it works:

  • After completing a short intake form, you’ll be matched with a peer based on shared experiences, background, and preferences.
  • You’ll meet virtually (or by phone) on a flexible schedule that works for both of you.
  • Peer supporters are trained to listen, support without judgment, and help you set recovery or care-related goals.

Why participate?

  • Gain strength and perspective from someone who understands your journey
  • Build a trusting relationship that fosters healing and resilience
  • Reduce feelings of isolation and burnout—whether you're a survivor or caregiver
